- Completed University of Pittsburgh Residency Certification Form/Address Change PDF
- Certificate of English Language Fluency PDF
- Email Address Form PDF
- DS-2019 Certificate of Eligibility for Exchange Visitor Status
- Most recent I-94
- Copy of Visa/Permanent Resident/Passport
NOTE: The Office of Trade Compliance (Chuck Lyon, the Visitor Liaison) must approve the MyEC record submission and then acquire a fully executed Visitor Participation Agreement before the appointment packet is submitted in One Drive for the initial appointment.
