Pitt/UPP Faculty

  1. Completed Employee Record (ER)
  2. Signed Faculty Compensation Change Form (salary decrease must include written explanation for the reduction)
  3. Calculations (if applicable)
  4. Service Request (if applicable)

ANY COMPLETED EMPLOYEE RECORD (ER) OR APPOINTMENT FORM MUST BE SUBMITTED WITH ALL SUPPORTING DOCUMENTATION AND ONE COPY OF ALL SUPPORTING DOCUMENTATION
