Administrative Checklists: Faculty Terminations

  1. Completed Employee Record (ER)
  2. Resignation letter
  3. If non-renewal, attach copy of non-renewal letter
  4. Partial month pay calculation (if applicable)
  5. Forwarding address
  6. Completed University of Pittsburgh Residency Certification Form/Address Change only if not changed via portal prior to the termination date 

ANY COMPLETED EMPLOYEE RECORD (ER) OR APPOINTMENT FORM MUST BE SUBMITTED WITH ALL SUPPORTING DOCUMENTATION AND ONE COPY OF ALL SUPPORTING DOCUMENTATION
