  2. Chair letter from the secondary department describing the basis for the secondary appointment or promotion
  3. Chair letter of concurrence from the primary department
  4. Current curriculum vitae

ANY COMPLETED EMPLOYEE RECORD (ER) OR APPOINTMENT FORM MUST BE SUBMITTED WITH ALL SUPPORTING DOCUMENTATION AND ONE COPY OF ALL SUPPORTING DOCUMENTATION
