Administrative Checklists: Tenure Stream Reappointments

  1. Completed Employee Record (ER)
  2. Signed Reappointment Letter
  3. Chair letter explaining the basis for continuation of the faculty member's contract and a description of departmental faculty committee deliberations
  4. Faculty member's annual review** and academic plans and measurable outcomes both must be signed by the department chair and the faculty member
  5. Current curriculum vitae
  6. Service Request (if past payroll deadline)
  7. Electronic Employment Eligibility Verification Form (I-9) Instruction(if applicable)

THE COMPLETED EMPLOYEE RECORD (ER) MUST BE SUBMITTED WITH ORIGINAL AND ONE COPY OF ALL SUPPORTING DOCUMENTATION **If the faculty member's prior contract was for 2 or 3 years, the annual review for those years must accompany the current annual review.
